Admission to HSA Southwest Chicago is a three-step process.
Each student/family who wants to attend HSA Southwest Chicago must submit an application by the deadline below. The application will be fairly simple, requesting basic biographical information (name, age, etc.), contact information, and other important facts (e.g., school currently attended).

If the number of eligible applications for admission exceeds the spaces available for students, a random lottery will be held. The lottery will be open to the public. It will be conducted before a notary public, and a representative from Chicago Public Schools will be invited. The lottery process will also be videotaped and archived. Siblings of Horizon Science Academy students will be given preference in order to keep families together.

Lottery will be by grade levels. Separate waiting lists will be maintained for each grade level in the event that vacancies occur. The random lottery process used for student admission will serve to place students in preferential order on the waiting lists. Whenever a vacancy occurs, either prior to the start of a particular school year or during the course of that school year, the school will contact the parent or guardian of the student next on the appropriate waiting list.
Reasonable and multiple attempts will be made to contact the family of the student on the top of the waiting list to receive confirmation of whether the student is still interested in enrolling at HSA Southwest Chicago before proceeding to the next name on the list.
Immediately after the lottery, Horizon Science Academies will inform all parents about the lottery results. Those whose names have been selected will receive a congratulation letter. The rest of the applicants will receive letters informing them that they will be on the waiting list and what their number on the waiting list is.
HSA Southwest Chicago will then give parents three weeks to enroll their children at the schools. Enrollment deadline is Wednesday, July 2, 2014.
Enrollment will require filling out the enrollment form, which is lengthier than the application form, submitting proof of residency, report cards from the previous schools, release forms, IEP documents if they exist, emergency contact information, a home language survey, and lunch application. The enrollment package will be mailed to the accepted students after the lottery.

Upon admission, HSA Southwest Chicago will organize small group orientations with students and parents. During orientations, we will meet with students and parents to ensure that parents’ and students’ questions are answered concerning expectations, promotion policies, grading, discipline policies, graduation requirements, college acceptance procedures, post-secondary options, advanced study groups, local and international trips, tutoring, and school events.
Parents will receive the Horizon Science Academy Student Handbook, which clearly explains all the details of the school’s academic and non-academic policies. They will be asked to sign the acknowledgement letter indicating their commitment to these policies.
Upon admission, students will also be required to take diagnostic tests created by Concept Schools. Diagnostic test results will help the principal and the instructional coordinator to understand each student’s needs in order to co-create a personalized education plan for their future academic success. The diagnostic tests will be only in mathematics and reading.
Once enrolled at HSA Southwest Chicago, students will not go through a lottery process again. However, each year they will have to complete re-enrollment forms indicating their intention to stay at Horizon Science Academy Clay Evans for the next school year. The re-enrollment deadline will be communicated to parents in different forms and multiple times in order to ensure that they are informed about the deadline. Communication will include letters sent home, newsletters, phone calls, meetings with students, and announcements.
Horizon Science Academies will charge a minimal fee to parents for consumable materials ($75) per child. However, such fee will be waived for eligible parents if they request a waiver in writing. The fee will be communicated to the parents after they are accepted to Horizon Science Academies and will not be a deterrent of admission or attendance.
As Horizon Science Academy students are required to wear uniforms, which they will be able to purchase at the school, the fee for uniforms will also be waived for eligible parents upon a written waiver request.
